How to use it
How to use the workbook
Start by reading the Instructions sheet, then enter one original appointment per row on the Appointments sheet. Use the provided dropdowns for standardized categories and complete Scheduled Start for each record. For a canceled appointment, enter its Cancellation Reason and Canceled At date and time. If it was rebooked, complete both replacement appointment fields. Next, filter the Data Quality Alert column for nonblank values and correct the underlying records. Finally, set the Start Date and End Date on the Dashboard. All Dashboard metrics use Scheduled Start to determine whether an appointment falls within the selected period. Do not enter patient names, initials, birth dates, medical record numbers, diagnoses, treatment details, or clinical notes.

Included features and controls
+Four-sheet workbook: Includes Dashboard, Appointments, Lists, and Instructions sheets.
+500 prepared appointment rows: The Appointments sheet is prepared from rows 6 through 505. Fictional sample records demonstrate the layout, while the remaining prepared rows support operational entry.
+Seven calculated output fields: Automatically derives Cancellation Lead Hours, Cancellation Lead-Time Band, Appointment Month, Canceled Flag, No-Show Flag, Rescheduled Flag, and Data Quality Alert.
Analyze Cancellation Reasons and Notice Periods
The Dashboard summarizes canceled appointments by reason, showing both counts and percentages for the selected period. For records with valid Scheduled Start and Canceled At values, the workbook calculates cancellation lead hours and assigns one of four bands: under 24 hours, 24 to under 48 hours, 48 to under 168 hours, or 7 or more days. These outputs support focused review of late cancellations without requiring patient or clinical information.

Track No-Shows, Rescheduling, and Avoidability
Appointment Status supports Scheduled, Completed, Canceled, No-show, and Other values. The Dashboard calculates the no-show rate separately from the cancellation rate. Replacement appointment fields feed a rescheduled cancellation rate, while the Avoidable Cancellation field supports an avoidable cancellation rate based on canceled appointments marked Yes.
Compare Appointment Cancellation Rates by Clinic
The clinic summary reports total appointments, canceled appointments, and cancellation rate for each clinic included in the prepared Lists range. A corresponding chart displays clinic cancellation rates. Because the analysis uses Scheduled Start, clinic results follow the same Start Date and End Date controls as the headline Dashboard metrics.

Review Data Quality Before Sharing Results
The calculated alert field helps surface records that need correction or confirmation. The Instructions sheet directs users to review duplicate Appointment IDs, missing required cancellation details, negative cancellation lead hours, and incomplete rescheduling information. The Dashboard also reports rows with alerts and reminds users to review the Appointments sheet before distributing results.
Common questions
About this template
The Appointments sheet contains 500 prepared rows, covering rows 6 through 505. The workbook includes fictional sample records that demonstrate the intended entry format; these samples should not be confused with operational clinic data.
The Dashboard uses Scheduled Start as its date basis. An appointment is included when its Scheduled Start falls within the Start Date and End Date entered on the Dashboard.
A same-day cancellation is a canceled appointment with a valid lead time from zero to less than 24 hours before Scheduled Start. Negative lead time is treated as invalid timing rather than a valid same-day cancellation.
Yes. Enter the Rescheduled Appointment ID and Rescheduled Start when an appointment is rebooked. The calculated Rescheduled Flag recognizes canceled records with replacement appointment information, and the Dashboard calculates the rescheduled cancellation rate.
The clinic summary shows total appointments, canceled appointments, and cancellation rate for clinics in the prepared Lists range. It also includes a chart based on clinic cancellation rates.
Do not enter patient names, initials, birth dates, medical record numbers, diagnoses, treatment details, or clinical notes. Operational Notes should contain only brief scheduling-process information. The workbook supports operational review and does not provide clinical, legal, payer, or regulatory advice.
