This free Excel template gives clinic operations teams a structured patient registration audit checklist for reviewing registration accuracy, recording findings, and tracking corrective action. Enter one audited registration per row, assess 10 checklist items, and use calculated outputs to monitor failures, completion, pass rates, follow-up alerts, and Dashboard scope.

The workbook contains four sheets—Dashboard, Audit Log, Lists, and Instructions—and provides 500 prepared Audit Log rows from rows 2 through 501. Use fictional patient references only; do not enter patient names or sensitive personal information.

In practice

What this template helps you manage

  • Standardize registration reviews: Apply the same 10-point checklist to each audited registration, with controlled choices of Pass, Fail, N/A, and Not Reviewed.
  • Identify recurring errors: Review failure counts and failure rates for legal name, date of birth, address, contact information, emergency contact, payer and plan, member ID, consent forms, primary care provider, and duplicate-record checks.
  • Monitor corrective work: Record an owner, due date, resolution status, and resolved date when a failed item requires follow-up.
  • Compare operational results: Use Dashboard date, clinic, and auditor selections to review audit volume, completion, failures, pass rates, and overdue follow-up within the chosen scope.

How to use it

How to use the workbook

Start by selecting a Dashboard start date, end date, clinic, and auditor; clinic and auditor can each be set to All. Next, enter one audited registration per Audit Log row using a fictional patient reference. Complete all 10 checklist fields with Pass, Fail, N/A, or Not Reviewed, and add concise notes for failed items. If correction is required, assign a follow-up owner and due date, then update the resolution status and resolved date when work is finished. Finally, review the Dashboard for incomplete audits, recurring failures, open corrective work, and overdue follow-up. Preserve the formulas in columns Y through AE when replacing or clearing the fictional sample inputs.

A Consistent Patient Registration Audit Checklist

The 10-item registration checklist creates a repeatable review structure for core registration fields and processes. Auditors can mark each item Pass, Fail, N/A, or Not Reviewed. A row is calculated as Incomplete when any checklist cell is blank or marked Not Reviewed; otherwise, it is calculated as Complete.

Automated Pass Rates and Failure Tracking

The Audit Log automatically counts failed items and reviewed items. Its calculated pass rate divides passed reviewed items by all items marked Pass or Fail, excluding N/A and Not Reviewed from that denominator. Dashboard summaries then show overall, clinic-level, and checklist-level results for audits matching the selected scope.

Corrective Follow-Up for Registration Errors

For audits with failures, users can document notes, assign an owner, enter a due date, and maintain the resolution status. The Follow-Up Alert can return Resolved, Due Date Missing, Overdue, Due Today, or Open based on the entered follow-up information and current date. Audits without failures return No Follow-Up Needed.

Clinic and Auditor Reporting in Excel

The Dashboard’s date, clinic, and auditor filters determine which Audit Log rows are marked In Scope. Teams can review summary indicators, compare clinic pass rates, and see checklist failure counts without manually rebuilding formulas. The workbook is an operational audit aid and does not represent a compliance determination.
