In practice

What this template helps you manage

  • Coordinate 500 prepared trip rows: The Transportation Log provides rows 2 through 501 for operational records. Eight formula-driven output columns are already prepared across the full range.
  • Monitor pickup and arrival timing: Calculated outputs combine service dates with entered times and report pickup delay, arrival versus appointment time, and on-time pickup results.
  • Identify follow-up needs: The workbook calculates a Review Alert for entered records and includes dashboard measures for unassigned vendors, pickups due within two hours, and overdue pickups.
  • Summarize filtered operations: The Dashboard reports trip volume, status, timing, costs, and clinic activity based on selected report dates, clinic, and coordinator.

How to use it

How to use the workbook

Start on the Dashboard and select the report start date, report end date, clinic, coordinator, and as-of date and time. Then enter one transportation leg per row in the Transportation Log and assign a unique Trip ID. Complete the applicable patient reference, clinic, appointment, mobility, vendor, pickup, status, coordinator, cost, contact, and notes fields. Enter outbound and return rides as separate records. Review the calculated date-time, delay, on-time, alert, and dashboard-eligibility columns without replacing their formulas. Return to the Dashboard to monitor matching trips, pickup exceptions, performance measures, costs, and chart summaries.

500 prepared operational rows in the Transportation Log
32 log columns covering trip, scheduling, vendor, status, cost, contact, and notes information
Eight calculated columns: Scheduled Pickup Date and Time, Actual Pickup Date and Time, Actual Arrival Date and Time, Pickup Delay Minutes, Arrival vs. Appointment Minutes, On-Time Pickup, Review Alert, and Dashboard Eligible

Patient transportation scheduling and trip-leg documentation

The log is organized around one transportation leg per row. Users can record a Trip ID, service date, appointment time, approved patient reference, clinic, appointment type, origin zone, mobility need, escort requirement, trip leg, vendor, booking reference, pickup window, status, coordinator, cancellation reason, costs, contact activity, and coordination notes. Outbound and return rides are separate records with separate Trip IDs, although the same approved patient reference can associate the legs.

Calculated pickup performance and review alerts

Eight prepared formula columns turn entered dates and times into operational timing outputs. The workbook calculates scheduled pickup, actual pickup, and actual arrival date-times; pickup delay in minutes; arrival versus appointment minutes; and an on-time pickup result. It also produces a Review Alert and determines whether the row matches the current dashboard criteria. The Lists & Settings sheet defines on-time pickup as an actual pickup no later than the scheduled pickup plus Pickup Window Minutes.

Clinic operations dashboard and transportation KPIs

The Dashboard provides a filtered transportation overview based on report start and end dates, clinic, and coordinator. Its editable as-of date and time controls the pickups-due-within-two-hours and overdue-pickup indicators. KPI areas cover trip status and workload, vendor assignment, pickup performance, late clinic arrivals, average pickup delay, estimated cost, and actual cost. Two charts summarize trip counts by status and by clinic.

Validated entry, privacy guidance, and workbook limits

Relevant inputs use Excel data validation for dates, times, text lengths, numeric ranges, and list selections. Pickup Window Minutes accepts whole numbers from 0 to 180, while estimated and actual costs accept values from 0 to 10,000. The Instructions sheet emphasizes data minimization: do not enter names, birth dates, diagnoses, medical record numbers, insurance identifiers, home addresses, phone numbers, or clinical notes. The workbook supports coordination and reporting only; it does not book transportation, verify benefits, authorize services, or determine medical appropriateness.
