Surface overdue follow-ups and closing appointment windows
The template calculates four operational outputs for each prepared request row. Days Waiting measures elapsed time from Requested On for records marked Active, Contacting, or On Hold, with a minimum value of zero. Follow-up Flag can identify Closed, Hold date missing, Hold expired, On hold, Follow-up date missing, Overdue, Due soon, or Planned conditions. Date Window Flag can identify Closed, Date missing, Expired, Closing soon, or Open conditions. The timing for Due soon and Closing soon is controlled on Lists & Setup.
Review clinic workload on the appointment waitlist dashboard
The formula-driven Dashboard reports open requests and scheduling priorities using current Waitlist records. Open Requests includes Active, Contacting, and On Hold statuses, while Active Requests counts only Active records. Other measures cover overdue follow-ups, closing and expired date windows, average open days waiting, high-priority open requests, and records scheduled during the last 30 days. Supporting summaries group open requests across eight listed services and five waiting bands: 0–7, 8–14, 15–30, 31–60, and over 60 days. Two charts visualize those summaries.

Standardize data entry while minimizing sensitive information
Dropdowns help staff use consistent clinic, service, appointment type, contact method, time preference, priority, status, and outcome values. Validated date fields accept dates from January 1, 2020, through December 31, 2100. The workbook also checks numeric limits and restricts Operations Note to 250 characters. Its Instructions sheet establishes a privacy-minimizing workflow: do not enter patient names, dates of birth, medical record numbers, telephone numbers, email addresses, mailing addresses, diagnoses, symptoms, treatment details, or other clinical information. Request Alias should be non-identifying, and Operations Note should contain only brief scheduling workflow information.
Common questions
About this template
The Waitlist sheet contains 200 prepared operational rows, covering rows 5 through 204. Each row is intended for one waitlist request.
The workbook includes populated sample records to illustrate data entry. Those samples occupy prepared rows and do not increase the workbook’s stated capacity.
Users enter or select request information in columns A through T. These fields cover the request ID and alias, clinic and service details, appointment preferences, acceptable date range, priority, status, outreach activity, scheduling outcome, and a brief operations note.
Columns U through X are calculated outputs: Days Waiting, Follow-up Flag, Date Window Flag, and Data Check.
Yes. Lists & Setup contains two configurable thresholds. Follow-up Due Soon Days is supplied as 3 and accepts a whole number from 0 to 30. Date Window Closing Soon Days is supplied as 7 and accepts a whole number from 0 to 60.
These settings control when a planned follow-up is labeled Due soon and when an open appointment window is labeled Closing soon.
The Dashboard shows Open Requests, Active Requests, Overdue Follow-ups, Windows Closing Soon, Expired Windows, Average Open Days Waiting, High-Priority Open Requests, and Scheduled in Last 30 Days.
It also provides summaries and charts for open requests by listed service and by waiting-time band. A data-review message indicates whether populated records have a Data Check result other than OK.
No. The workbook is intended for non-identifying operational scheduling information. Its instructions prohibit patient names, dates of birth, medical record numbers, telephone numbers, email addresses, mailing addresses, diagnoses, symptoms, treatment details, and other clinical information.
Use Request Alias only as a non-identifying workflow reference, and limit Operations Note to brief scheduling workflow information.
