In practice
What this template helps you manage
✓
Make shift handoffs easier to review: Record one operational handoff item per row so the incoming team can see what was received, who owns it, its current status, and what should happen next.
✓
Identify time-sensitive work: The calculated Due Alert classifies open items as Overdue, Due today, Due within 24 hours, Upcoming, or No due date based on the current date and time.
✓
Improve accountability: Use the Assigned To, Status, Last Updated, Follow-up Needed, and Next Step / Notes fields to keep responsibility and progress visible.
✓
Find entry problems before review: The calculated Review Check can surface issues such as duplicate IDs, missing required fields, completion inconsistencies, and a due date earlier than the logged date.
How to use it
How to use the workbook
Start on the Dashboard and set the Review Date, Clinic Filter, and Assigned To Filter. In the Handoff Log, enter one operational item per row and assign a unique Handoff ID using the format described in the Instructions sheet. Complete the applicable user-entry fields, including clinic, category, urgency, handoff item, owner, and status. Add a due date when the next action is time-sensitive, update Last Updated whenever work advances, and document the next step. When work is finished, set the status to Completed and enter Completed Date/Time. Before each shift change, review open, overdue, unassigned, high-urgency, and data-issue counts, filter the log as needed, and correct any Review Check result that is not OK.

What the front desk handoff log records
The Handoff Log provides fields for the handoff ID, logged date and time, clinic, source, category, urgency, operational reference, handoff item, assigned role, due date and time, status, last update, completion time, follow-up requirement, and next step. This gives front desk teams a consistent place to capture operational ownership and next actions without turning the workbook into a clinical record.

How the Excel dashboard supports daily review
The Dashboard uses the selected review date to summarize records logged before the end of that date. Clinic and Assigned To filters can narrow the results, while the All option preserves a combined view. Its metrics highlight total activity, currently open work, overdue work, work due on the review date, completed items, unassigned items, data issues, and open high-urgency items. Status and open-urgency summaries feed two dashboard charts.
Built-in due alerts and review checks
For open records, Due Alert can return No due date, Overdue, Due today, Due within 24 hours, or Upcoming. Age in Days measures elapsed time from Logged Date/Time to Completed Date/Time, or to the current time when no completion date exists. Review Check examines record quality and consistency, helping staff focus on entries that need correction before or during handoff review.

Standardized lists, statuses, and urgency levels
The Lists sheet supplies the workbook’s selectable values, including clinics, handoff sources, categories, assigned roles, Yes/No choices, five statuses, and three urgency levels. Status choices are New, In Progress, Waiting, Completed, and Canceled. Urgency choices are High, Standard, and Low. A separate setting stores the stale-open-item threshold in hours and accepts a whole number from 1 through 720, supporting consistent operational classification.
Common questions
About this template
The Handoff Log contains 250 prepared operational rows, spanning rows 6 through 255. The workbook includes populated sample records to illustrate the intended entry format; these are examples rather than additional capacity.
The calculated columns are Age in Days, Due Alert, and Review Check. They contain formulas throughout the prepared range. The other 15 columns are user-entry fields to complete when applicable.
You can select a Review Date and filter the dashboard by clinic or assigned role. Both list filters include an All option. The Review Date is validated for dates from January 1, 2020 through December 31, 2100.
The Review Check evaluates conditions including duplicate IDs, missing required fields, a completed status without a completion date, a completion date with a status other than Completed, and a due date before the logged date. The Dashboard counts entries whose Review Check is populated and is not OK.
Status choices are New, In Progress, Waiting, Completed, and Canceled. Urgency choices are High, Standard, and Low. Their operational definitions are documented on the Instructions sheet.
No. The workbook states that users should not enter patient names, dates of birth, diagnoses, clinical details, full medical record numbers, insurance member IDs, payment card data, passwords, or other sensitive information. Use it for operational references and administrative next steps rather than clinical documentation.
