This free Excel template is a patient payment plan tracker designed for administrative use in clinics and medical offices. The workbook connects payment plan records with a separate payment transaction log, then calculates scheduled amounts, posted payments, outstanding balances, past-due amounts, aging, next due dates, and plan statuses.
It includes five sheets—Dashboard, Payment Plans, Payments, Setup, and Instructions—and uses account aliases instead of patient names. Prepared ranges provide 250 payment plan rows and 1,000 payment transaction rows, including populated samples that can be replaced with approved administrative records.

How to use it
How to use the workbook
Start on Setup by entering the As of Date and a Due Soon Window from 0 to 60 days. Add one arrangement per row on Payment Plans, using a unique Plan ID and an approved account alias rather than a patient name. Enter the plan record status, clinic, responsible team, start date, original amount, monthly installment, and optional follow-up details. Next, record one transaction per row on Payments and connect it to an existing Plan ID. Select Posted or Void, enter the payment date and amount, and add the permitted administrative references. Review Row Check, Duplicate Check, and Transaction Check results, then use the Dashboard to assess balances, aging, due-soon activity, clinic totals, charts, and review alerts. Only Posted transactions contribute to payment totals; Void transactions stay visible but are excluded.

Track Payment Plans, Installments, and Balances
The Payment Plans sheet keeps each administrative arrangement on one row. User inputs include Plan ID, Account Alias, Plan Record Status, Clinic, Responsible Team, Plan Start Date, Original Plan Amount, Monthly Installment, Follow-Up Owner, Next Follow-Up Date, and Administrative Note. Formula-driven fields calculate the installment count, contract end date, scheduled amount, posted payments, outstanding balance, and past-due amount. This separation makes the workbook useful as a patient payment plan tracker without turning it into a clinical record or payment-processing system.

Review Overdue and Due-Soon Accounts as of a Chosen Date
The reporting logic follows the As of Date entered on Setup. A plan is Overdue when its scheduled amount as of that date exceeds its Posted payments as of that date. Overdue plans are assigned to 1–30, 31–60, 61–90, or 91+ day aging buckets. A plan that is not overdue can be classified as Due Soon when its next due date falls inside the configured 0-to-60-day window. Plans may also appear as Current, Paid, Starts Later, or Closed With Balance according to the workbook formulas.
Maintain a Linked Payment Transaction Log
The Payments sheet records Payment ID, Plan ID, Payment Date, Payment Amount, Payment Status, Payment Method, Receipt Reference, Entered By, and Administrative Note. Account Alias and Clinic are calculated from the matching Plan ID. Only transactions marked Posted are included in payment totals, while Void transactions remain visible and are excluded. A Posted payment dated after the As of Date is excluded from as-of reporting and flagged as a future-dated payment for review.

Use Dashboard Summaries and Data-Quality Checks
The Dashboard brings together plan counts, financial totals, collection progress, aging, statuses, clinic summaries, and two charts. Its Review Alerts area counts invalid plan rows, invalid payment rows, duplicate Plan IDs, payments with unknown Plan IDs, and future-dated Posted payments. The underlying sheets also detect duplicate Payment IDs. For appropriate data minimization, use approved account aliases and do not enter patient names, dates of birth, diagnoses, treatment details, insurance-member identifiers, payment-card numbers, bank-account numbers, Social Security numbers, addresses, or other unnecessary sensitive information.
Common questions
About this template
The prepared operational ranges contain 250 payment plan rows and 1,000 payment transaction rows. Some initial rows contain populated sample records for demonstration, and those samples are part of the prepared capacity rather than additional rows.
A plan is classified as Overdue when the scheduled amount as of the configured reporting date exceeds Posted payments as of that date. The workbook then calculates the past-due amount, aging days, and the applicable 1–30, 31–60, 61–90, or 91+ day aging bucket.
Only transactions marked Posted are included in payment totals. Void transactions remain visible but are excluded. Posted payments dated after the As of Date are excluded from as-of reporting and flagged for review.
Yes. Enter a Due Soon Window from 0 to 60 days on Setup. A plan that is not overdue is marked Due Soon when its calculated next due date falls between the As of Date and the end of that window.
The workbook checks for incomplete or invalid plan and payment rows, duplicate Plan IDs, duplicate Payment IDs, payments linked to unknown Plan IDs, and future-dated Posted payments. Related alert counts appear on the Dashboard.
No. This is an administrative tracker intended for fictional or approved account references. Use account aliases and do not enter patient names, clinical information, card numbers, bank-account numbers, Social Security numbers, or other unnecessary sensitive data.
