This free Excel template provides a structured claim rejection worklist for clinic operations and billing follow-up in 2026. Use the claim rejection tracker to assign owners, record priorities and statuses, monitor due dates, document administrative next actions, and review unresolved billed amounts. The workbook contains a Dashboard, Worklist, Lists, and Instructions sheet.
Its Worklist has 200 prepared operational rows, including fictional sample records that can be replaced or removed. It is intended for data-minimized administrative tracking and instructs users not to enter patient identifiers or clinical details.
In practice
What this template helps you manage
- Prioritize time-sensitive records: Calculated due alerts classify entered records as Closed, Missing Due Date, Overdue, Due Soon, or On Track. The Dashboard also counts overdue and due-soon open records using its configurable As of Date and due-soon window.
- Assign clear responsibility: Track an owner, priority, status, follow-up due date, and next administrative action for every entered rejection record. Filters make it possible to focus the Worklist by owner, status, priority, payer, category, or due alert.
- Monitor unresolved billed amounts: The Dashboard calculates the billed amount associated with open records and the open amount associated with overdue records. These are operational totals based on values entered in the Worklist.
- Identify incomplete worklist entries: The calculated Data Check field evaluates entered rows for duplicate Worklist IDs and missing operational fields. A Dashboard metric counts records whose Data Check result is not OK.

Common questions
About this template
The Worklist contains 200 prepared operational rows, covering rows 5 through 204. The fictional sample records occupy some of those rows and are not additional capacity. They can be replaced or removed before operational use.
Yes. For an entered record that is not marked Resolved, the Due Alert formula returns Overdue when the Follow-up Due Date is earlier than the Dashboard As of Date. It can also return Due Soon, On Track, or Missing Due Date; resolved records return Closed.
Yes. The Dashboard validates the Due Soon Window as a whole number from 0 to 30 days and the Aging Review Threshold as a whole number from 1 to 365 days. The supplied values are 3 days and 14 days.
Dropdown validations are provided for Clinic, Payer, Rejection Category, Owner, Priority, and Status across rows 5 through 204. Their choices come from fixed ranges on the Lists sheet.
Do not enter patient names, dates of birth, medical record numbers, diagnoses, procedure details, clinical notes, or copies of payer correspondence. Use an internal, non-patient claim reference and limit Notes to necessary administrative actions.
The Dashboard contains formulas linked to the prepared Worklist range, and its two charts reference dashboard summaries for status and rejection category. In an Excel environment that recalculates formulas, results reflect the entered Worklist data and the Dashboard settings.
