This free Excel template helps organize accounts receivable aging for a medical practice in 2026. Enter one accounts receivable record per row to calculate current balances, days outstanding, aging buckets, priorities, and data-quality alerts.

The four-sheet workbook supports up to 500 prepared AR rows and provides dashboard summaries for positive AR, credit balances, denials, follow-ups, payers, clinics, and aging periods. Included sample records are fictional and demonstrate the layout; they are not patient or production data.

In practice

What this template helps you manage

  • See the age of positive AR: Review positive accounts receivable across five standard periods: 0–30, 31–60, 61–90, 91–120, and over 120 days. The Dashboard also calculates the percentage of positive AR that is over 90 days.
  • Create a focused follow-up list: Track claim status, denial flag, last follow-up date, next action date, owner, and notes. A calculated AR priority classifies each entered record as Closed/Credit, Critical, High, Medium, or Routine.
  • Review payer and clinic exposure: Compare positive AR and open record counts for the included payer and clinic lists. Two Dashboard charts display positive AR by aging bucket and by the seven listed fictional payers.
  • Separate open balances from credits: Current Balance equals Original Charge minus Payments minus Adjustments. Positive balances contribute to open AR reporting, while negative balances appear separately as credit balances.

How to use it

How to use the workbook

Start on the Dashboard and enter the as-of date, select Service Date or Claim Submitted Date as the aging basis, set a nonnegative high-balance threshold, and choose a follow-up alert period from 0 to 365 days. Next, enter one record per row on AR Detail using the user-entry columns; do not replace formulas in the calculated columns. Review Current Balance, Days Outstanding, Aging Bucket, Priority, and Data Quality Alert, then use the Dashboard to examine aging, denials, overdue or missing follow-ups, payer totals, clinic totals, and credits. Finally, review Lists & Checks and resolve any status marked for review before relying on the summary. The prepared range runs from row 2 through row 501, providing capacity for up to 500 AR records.

Included features and controls

Four purpose-built worksheets: The workbook contains Dashboard, AR Detail, Lists & Checks, and Instructions sheets.
Configurable aging basis: Choose Service Date or Claim Submitted Date. When claim-date aging is selected but a claim submission date is blank, the formula uses Service Date instead.
Automatic balance and aging calculations: Prepared formulas calculate Current Balance, Aging Start Date, Days Outstanding, Aging Bucket, and an amount for each of the five aging periods.
Operational Dashboard metrics: Metrics include Total Positive AR, Open AR Records, AR Over 90 Days, Credit Balances, Denied Positive AR, Follow-Ups Due, Critical Records, Data Quality Issues, Average Positive Balance, Percent Over 90 Days, No Follow-Up Scheduled, and Records Over 120 Days.

Medical Practice AR Aging by Service Date or Claim Date

The medical practice AR aging logic is controlled from the Dashboard. Select Service Date to age balances from the date of service, or select Claim Submitted Date to use the submitted claim date when one exists. Days Outstanding is calculated through the selected as-of date, with negative results limited to zero. Each record is then assigned to 0–30, 31–60, 61–90, 91–120, or over 120 days.

Accounts Receivable Follow-Up and Denial Tracking

AR Detail combines financial fields with practical follow-up information. Users can enter Claim Status, Denial Flag, Last Follow-Up Date, Next Action Date, Owner, and Notes. The Dashboard highlights denied positive AR, follow-ups due on or before the as-of date, positive-balance records without a scheduled next action, and records classified as Critical.

Payer, Clinic, and Aging Dashboard Reporting

The Dashboard summarizes positive accounts receivable by aging bucket, listed payer, and clinic. Payer and clinic sections show positive AR and open record counts. The two included charts visualize positive AR across the five aging buckets and the seven listed fictional payers. Dashboard totals remain zero when no qualifying records exist.

Excel AR Data Quality and Reconciliation Checks

The workbook supports a more controlled review process through data-quality and reconciliation checks. Each entered AR Record ID is checked for duplicates, and required-field alerts cover clinic, account ID, payer, service date, and original charge. Lists & Checks also compares total Current Balance with the five aging amount columns and compares Dashboard positive AR with the corresponding positive balances on AR Detail. The workbook does not include macros, Excel tables, conditional formatting, or external integrations.
