This free Excel template provides a structured claim rejection worklist for clinic operations and billing follow-up in 2026. Use the claim rejection tracker to assign owners, record priorities and statuses, monitor due dates, document administrative next actions, and review unresolved billed amounts. The workbook contains a Dashboard, Worklist, Lists, and Instructions sheet.

Its Worklist has 200 prepared operational rows, including fictional sample records that can be replaced or removed. It is intended for data-minimized administrative tracking and instructs users not to enter patient identifiers or clinical details.

In practice

What this template helps you manage

  • Prioritize time-sensitive records: Calculated due alerts classify entered records as Closed, Missing Due Date, Overdue, Due Soon, or On Track. The Dashboard also counts overdue and due-soon open records using its configurable As of Date and due-soon window.
  • Assign clear responsibility: Track an owner, priority, status, follow-up due date, and next administrative action for every entered rejection record. Filters make it possible to focus the Worklist by owner, status, priority, payer, category, or due alert.
  • Monitor unresolved billed amounts: The Dashboard calculates the billed amount associated with open records and the open amount associated with overdue records. These are operational totals based on values entered in the Worklist.
  • Identify incomplete worklist entries: The calculated Data Check field evaluates entered rows for duplicate Worklist IDs and missing operational fields. A Dashboard metric counts records whose Data Check result is not OK.

How to use it

How to use the workbook

Start by updating the Dashboard As of Date, which controls aging and deadline calculations. You can also set the Due Soon Window from 0 to 30 days and the Aging Review Threshold from 1 to 365 days. On the Worklist, add one rejected claim per row using a unique Worklist ID and an internal, non-patient claim reference. Select the clinic, payer, rejection category, owner, priority, and status from the supplied dropdowns, then enter the applicable dates, billed amount, next action, and concise administrative notes. Review Age, Days Until Due, Due Alert, and Data Check before filtering the queue for follow-up. When corrected work is sent, enter the Resubmitted Date. When no additional rejection follow-up is required, change the status to Resolved and enter the Resolved Date.

Included features and controls

Four-sheet workbook: Includes Dashboard, Worklist, Lists, and Instructions sheets.
200 prepared Worklist rows: Rows 5 through 204 are prepared for records. Some are populated with fictional examples; those examples are part of the 200-row capacity rather than additional rows.
Sixteen user-entry fields: Enter Worklist ID, Clinic, Claim Reference, Payer, Rejection Code, Rejection Category, Rejection Received Date, Billed Amount, Owner, Priority, Status, Follow-up Due Date, Resubmitted Date, Resolved Date, Next Action, and Notes.
Four calculated fields: Formula-filled columns calculate Age (Days), Days Until Due, Due Alert, and Data Check across all 200 prepared rows.

Manage a daily claim rejection work queue

The Worklist combines classification, ownership, deadlines, and next actions in one filterable range. Staff can use owner and status filters to create focused queues without restructuring the workbook. Available statuses are New, Researching, Ready to Resubmit, Resubmitted, On Hold, and Resolved. Priorities are Low, Normal, High, and Urgent.

Track aging, due dates, and resubmissions

Age is calculated from the Rejection Received Date to the Resolved Date or, if no resolved date is entered, to the Dashboard As of Date. Days Until Due compares an open record’s Follow-up Due Date with that same As of Date. This makes the Dashboard As of Date an important review setting; the formulas do not automatically substitute the computer’s current date. Resubmitted and resolved dates can be recorded separately as work progresses.

Review workload and billed amounts on the dashboard

The Dashboard provides operational counts for open, overdue, due-soon, older, high-priority, resubmitted, recently resolved, and data-issue records. It also reports open billed amount, average open age, and the billed amount associated with overdue open records. Two charts summarize status counts and open counts across the nine supplied rejection categories.

Maintain consistent dropdown choices and safer records

The Lists sheet supplies five clinics, six payers, nine rejection categories, six owners, four priorities, and six statuses. These labels include fictional examples. Choices can be added below a list, but the corresponding validation source range must be updated manually. For safer operational tracking, use an internal non-patient claim reference and keep Notes limited to the administrative action required.
