In practice

What this template helps you manage

  • Centralize internal claim identifiers, service dates, submission details, payer activity, financial amounts, denial information, ownership, and next actions.
  • Review total claims, total billed, insurance paid, outstanding balance, denied claims, denial rate, follow-up workload, timely filing risk, data issues, paid claims, average days to payment, and claims ready to submit.
  • Use calculated Aging Days, Outstanding Balance, Age Bucket, Timely Filing Alert, Follow-Up Alert, and Data Quality Check results instead of manually rebuilding those calculations.
  • Prioritize unresolved work through aging, claim-status, and payer-level summaries on the Dashboard.

How to use it

How to use the workbook

Begin on Lists & Settings and review the available clinics, payers, claim types, claim statuses, denial categories, owners, and operational settings. In Claims Tracker, enter one claim per row using the blue input cells and internal identifiers only; do not enter patient names or member IDs. Add the service date, filing deadline, billed amount, and other applicable details, then update submission, payer response, payment, patient responsibility, adjustment, denial, follow-up, status, and owner fields as activity occurs. Review the Timely Filing Alert, Follow-Up Alert, and Data Quality Check columns for work that needs attention. Finally, use the Dashboard metrics, aging summary, status summary, payer summary, and charts to prioritize recurring claim reviews.

Track medical claims, payments, and open balances in Excel

The Claims Tracker brings operational and financial claim details into one structured worksheet. Teams can record billed and allowed amounts, insurance payments, patient responsibility, and adjustments. The calculated Outstanding Balance subtracts insurance paid, patient responsibility, and adjustments from the billed amount and does not return a result below zero. Dashboard totals then summarize billed amounts, insurance payments, and remaining balances from the tracker entries.

Monitor timely filing deadlines and payer follow-up

The workbook separates submission risk from post-submission follow-up. For unsubmitted Draft and Ready to Submit claims, the timely filing review uses the entered deadline and the warning-day setting to return Missing deadline, Past due, Due within 14 days, On track, or Not applicable. For claims with open balances, the follow-up calculation identifies No follow-up date, Overdue, Due, or Scheduled; zero-balance claims are labeled Closed.

Review aging, status, denials, and payer workload

Aging Days measures time from the Submitted Date through today, or through Last Payer Response for a Paid claim when that date is present. Claims are grouped as Not submitted, 0–30 days, 31–60 days, 61–90 days, or More than 90 days. The formula-driven Dashboard also summarizes claim counts and outstanding balances by status, reports denied claims and denial rate, and provides an outstanding-by-payer view.

Improve consistency with validations and data checks

Dropdowns guide entries for clinics, payers, claim types, statuses, denial categories, and owners, while date and nonnegative decimal validation support consistent input. The calculated Data Quality Check returns the first missing or inconsistent item detected in a populated row. Blue cells identify user inputs, gray cells identify calculated outputs, and yellow or red formatting marks items that may require review. Editing a list does not automatically revise existing claim entries, so the tracker should be reviewed after list changes.
