About this template
The Audit Log contains 500 prepared operational rows, covering rows 2 through 501. Formulas in the seven calculated columns and the applicable dropdown validations extend through those rows.
The available checklist statuses are Pass, Fail, N/A, and Not Reviewed. A blank or Not Reviewed checklist entry causes the calculated Audit Completion field to return Incomplete.
The row-level pass rate equals passed reviewed items divided by all reviewed items. Reviewed items are those marked Pass or Fail; N/A and Not Reviewed are not included in that denominator.
Yes. The Dashboard includes start-date and end-date inputs plus clinic and auditor dropdowns. Clinic and auditor can each be set to All, and only matching Audit Log rows are marked In Scope.
Yes. When an audit has failures, the calculated Follow-Up Alert evaluates the resolution status, follow-up due date, and current date. Depending on those entries, it can show Resolved, Due Date Missing, Overdue, Due Today, or Open.
Yes. The Lists sheet contains editable source lists for clinics, auditors, registration channels, checklist statuses, audit types, resolution statuses, and follow-up owners. Edit only the designated light-yellow list cells, keep entries unique, and leave unused rows blank.
